What Happens Before And During Surgery?
What occurs will depend on the extent of the work required.
For instance, a transgender patient may desire a more feminized nose while making their transition, but may also have other underlying problems that need to be rectified. Dr. Solomon may look at the basal frontline photo and discover that his patient has a chronic nasal blockage conundrum that needs to be treated immediately. Perhaps a look at the patient’s history may uncover a history of several nasal fractures brought on by high intensity contact sports. Aesthetically speaking, the patient may have an over-projected tip that is quite full in appearance as well as a broad hump on the bridge. In such a case, Dr. Solomon may begin with open Rhinoplasty incisions. This is done so that the patient’s deviated septum can be repaired. The nasal skin tip may also need to be elevated momentarily so that he can access the underlying nasal cartilage. A three point counter retraction technique may also be implemented in order to work on the nasal tip domes. Next, once the skin has been lifted, Dr. Solomon may use a scalpel to shave down any excess cartilage. The bony component of the bridge will then be brought down using a surgical apparatus called a oseteotome. Dr. Solomon will then check to ensure that the new profile line is where it should be, and he may then proceed to further smooth out the edges using a bone file. This method, known in the field as “rasping” allows Dr. Solomon to fine tune the height of the nasal bridge and ensure that no sharp bone edges are left. Such edges may disrupt the outward appearance of the nasal bridge and should be avoided at all costs. The goal is always to reshape while ensuring and maintainting a clear nasal airway. A secondary inspection of the bridge will then be performed in order to ensure an immaculate result. In the event that the patient has a problem with a deviated septum, Dr. Solomon may dichotomize the medial crura that make up the nasal tip columella. By doing so, he will have access to septum’s frontal edge allowing him to perform a septoplasty. A septoplasty is performed by elevating the mucosa or the internal skin lining off of the septum. As can be seen, the total needs of the patient are taken into account. If more than one surgical procedure is required to make a transition, Rhinoplasty is often done first and can be done in conjunction with lip augmentation and injectable fillers. Many patients find that this approach provides a significant enhancement to their looks early in their transition. As their transition progresses, patients may then choose to pursue laryngeal shave and finally, larger procedures like forehead reshaping. “I usually see patients who are making the transformation from male to female. A female nose has narrower nasal passages, a steeper nasal bridge and narrower nasal bones,” says Dr. Solomon. The Rhinoplasty takes place in hospital and is usually done under general anesthesia, taking 1 ½ to 2 hours to complete. Since the surgery is performed under general anesthesia you will not be aware of what is happening and will be perfectly comfortable and pain free throughout the entire surgical procedure. Depending on the size and shape of your nose, Dr. Solomon may shape and reposition the nasal bones, reduce the size of the bridge and slightly lift up the nose tip to create a more feminine look. The incisions are usually made inside the nostrils but even if external incisions are necessary, they will heal with nearly invisible scars.How Soon Will My Nose Recover?
After the surgery, a splint will be placed beside your nose for a week to ten days to give it support as it heals. In some cases, a light dressing may be placed inside the nostrils if there is any bleeding and then removed the same day or the next day. The internal sutures dissolve and fall out after about two weeks and if they were used, any external sutures will be removed in a follow-up appointment about one week after surgery. Your skin may feel numb after Rhinoplasty Surgery and there may be some bruising or residual swelling, especially in the nasal tip. Dr. Solomon may prescribe painkillers or you may be able to take a non-prescription painkiller, depending on the extent and complexity of the surgical procedure. It is always advisable to follow all after care instructions provided to ensure an uncomplicated and smooth recovery.
